This website introduces QM Care’s Occupational Therapy and Speech Pathology services. General information does not replace an individual assessment or medical advice. For emergencies, call 000.
Referrals and appointments
Submitting a referral allows our team to review the request. It does not confirm eligibility, an appointment or a service agreement. We aim to reply within 2 office hours and offer an appointment within 2 weeks, subject to service suitability and availability. We confirm appointments with you separately.
Fees, funding and cancellations
Before services begin, we discuss the scope of support, applicable fees, travel, reports and any cancellation arrangements in your service agreement. Applicable NDIS pricing rules and limits guide our fees. Please contact us if available funding is limited so we can discuss suitable options with you and, with consent, your support team. Extra funding is not guaranteed.
Consent and documents
Provide accurate information and only documents relevant to the referral. Obtain the participant’s consent or appropriate authority before sharing personal and health information. The form accepts up to five PDF, JPG or PNG documents, totalling 10 MB.
Privacy and delivery
Our Privacy Policy explains referral processing, email delivery, temporary encrypted storage and anonymous website statistics. Keep the reference shown after submission. If you do not hear from us, call 0433 996 736.
